A Department of Transportation (DOT) physical is a physical exam that is required of people who drive commercial vehicles. The point of a DOT physical is to make sure that people who are driving commercial motor vehicles (CMVs), such as large trucks and buses, are physically, mentally, and emotionally fit to safely operate their vehicles, protecting both themselves and the public from potential accidents or health-related emergencies on the road. If you are designated as “safety-sensitive” for the Department of Transportation (DOT) – meaning your job can affect both your own safety and the safety of the public – Such persons are required to have a regular physical to be compliant and keep working. A DOT physical follows strict guidelines mandated by the Federal Motor Carrier Safety Administration (FMCSA), ensuring that commercial drivers and other safety-sensitive employees are in good health to work and can safely perform their assigned duties.


What Does a DOT Physical Exam Check?
The D.O.T. Physical Exam checks:
Weight
Blood pressure
Temperature
Heart Rate
Ears, Eyes, Nose, Heart, Lungs, Throat, Abdomen, skin
Neurological and musculoskeletal systems
Genital exam (for men, if necessary)
Sleep Apnea And DOT Physical
Sleep apnea can impact a driver’s ability to safely operate a commercial vehicle. The Federal Motor Carrier Safety Administration (FMCSA) sets medical standards for drivers, including those for sleep apnea.
Obstructive Sleep Apnea (OSA): The most common type, causing repeated pauses in breathing during sleep.
FMCSA Guidance:Provides rules for evaluating and managing sleep apnea in drivers.
DOT Physical Exam:Requires a Certified Medical Examiner (CME) to confirm medical fitness.
Sleep Apnea Testing: Includes polysomnography (PSG) or home sleep tests (HST) for at-risk drivers.
Treatment and Certification: Drivers with sleep apnea need treatment (e.g., CPAP therapy) and must demonstrate condition control.


CPAP Use Requirements for DOT Certification
Drivers using a CPAP machine must:
Use the CPAP machine for at least 4 hours per night.
Use the CPAP machine 70% of the time (e.g., 21 out of 30 days per month).
